We are now taking enrollments for the 2009-2010 season. The season begins Monday August 3, 2009. To reserve a spot in the class(es) of your choice follow these steps:

• Fill out the lower portion of this form.
• Enclose a $40 enrollment check.
• Return form and check to Duo Dance Academy at 12 Wellington Ave. – Daly City, CA 94014

On the first day of classes there are enrollment and insurance forms to complete, and the tuition payments begin. If you have any questions please contact the Duo Dance at: (650) 992-2205.

Enrollment is on a first come first serve bases, some classes fill quickly; mail registration promptly.

Some classes may be subject to change.
